Refer a Patient

Referrals from Mecklenburg County

NC MedAssist takes new referrals from residents of Mecklenburg County by appointment only. Patients can call 704-536-1790 or 704-391-6603 to schedule an appointment.  Please bring your completed application to your appointment. If you are a resident of Mecklenburg County, click HERE for your application.  Application in Spanish HERE.

Patients should bring the following items to their appointment:

  • Last month’s check stubs
  • Latest Federal tax return
  • Written prescriptions (Rx) from the doctor
  • Completed application

If you do not have these documents, please call 704-536-1790 to speak with a social worker.

Patients must meet the following eligibility requirements:

  • Be a North Carolina resident
  • Cannot have Medicare, Medicaid or any other prescription drug insurance
  • Income must be at or below 200% of the Federal Poverty Level

Referrals from outside Mecklenburg County

A patient will access the central fill pharmacy program at NC MedAssist by calling 1-866-331-1348 or going to a doctor’s office, free clinic, community health center or the local health department in their county. The process begins by the patient obtaining a valid prescription for medication offered on the NC MedAssist formulary.

If the doctor’s office is not the POE, then the patient is referred to the POE to complete the eligibility information. The eligibility process will involve the POE working with the patient to complete a brief enrollment form, provided to the POE by NC MedAssist at the time the POE signs the contract for prescription delivery. The POE will fax to NC MedAssist the completed form. Upon receipt of the faxed enrollment form, NC MedAssist's service staff will call the patient and provide information on the enrollment process. The patient will need to either mail or fax in proof of income, most recent tax return and the prescription.

The Customer Service Representative will complete a short health survey upon enrollment and annually thereafter. When NC MedAssist receives the patient’s financial documents, application and prescriptions, the pharmacy will begin to fill the prescriptions. The pharmacist will review the medications ordered with other known medications the patient is taking. While the pharmacist completes the filling process, if there are any indications that further medication management is needed, then the patient and/or their provider will be called. If no additional management is needed, then the drugs are shipped the following Wednesday to the patient or point of entry in the county.

For new patients, NC MedAssist generally ships 30-day supplies initially and then 90-day supplies as refills are needed. At the time the patient receives the medications, a toll-free number is included with their prescription to call for counseling by a pharmacist. If patient is picking up at the POE, then the patient must present ID and sign for their medication.  

For an existing patient, when a new prescription is received from their doctor, the doctor’s office may fax it directly into NC MedAssist. The pharmacist then reviews the medication and completes the filling process as appropriate. Medication is shipped back to either the patient or the POE.
